Services & Care
Multidimensional Assessment and Treatment Planning
We conduct thorough assessments to understand each individual’s mental, emotional, and behavioral health. This guides the development of a personalized treatment plan tailored to their specific needs and goals.
Evaluation and Monitoring of Mental Health Status
Ongoing evaluations help us track progress and adjust care as needed. This ensures individuals receive the most effective support throughout their treatment.
Referral for Health Care and Support Services
When additional services are needed, we connect clients with trusted healthcare providers and community resources. These referrals help support overall wellness beyond behavioral health.
Implementation, Monitoring, and Evaluation of Treatment Plan
We put each treatment plan into action with consistent follow-up and care. Progress is closely monitored to keep the plan effective and aligned with changing needs.
Assistance in Obtaining Resources
Our team helps individuals secure essential resources like housing, food, transportation, or benefits. These supports lay the groundwork for long-term stability and recovery.
Aid in Increasing Community Access
We assist clients in becoming more engaged in their communities, whether through education, employment, or social programs. This fosters independence and a sense of belonging.
Psychotherapy & Counseling
Licensed clinicians provide one-on-one or group counseling to address mental health and substance use concerns. Therapy is customized to support emotional healing, personal growth, and coping strategies.
Crisis Planning
We help develop proactive plans for managing potential mental health crises. These plans offer clear steps and supports to ensure safety and stability in difficult moments.
Staffing with Medical Providers
Coordination with psychiatric and medical professionals ensures clients receive well-rounded care. This may include medication management or other medical support as needed.
Advocacy
We stand alongside our clients to help them voice their needs and protect their rights. Advocacy may involve navigating systems, accessing services, or addressing barriers to care.
Service Coordination
Our team works collaboratively across providers and systems to ensure seamless support. This helps reduce gaps in care and keeps services organized and aligned.
Discharge Planning
As individuals prepare to transition from our care, we help create a clear and supportive exit plan. This includes connecting them with continued services and resources to maintain progress.
